This FREE Advance Care Planning and Education Clinic will aid you and your family in planning for your future medical care. The educational presentation will provide information about the advance care planning process and the importance of making your medical choices known. Certified Respecting Choices facilitators and notaries will be available during the clinic to assist in the preparation of living wills and healthcare power of attorney documentation.
