Join us for FREE Advance Care Planning and Education Clinics to aid you & your family in planning for your future medical care. Certified Respecting Choices Facilitators and notaries will be available during the clinic to assist in the preparation of living wills and healthcare power of attorney documents. 9am will be the Education presentation and 10am will be the clinic.
Registration is preferred for completion of documents, but not required.
